This care plan is based on the available evidence on how to best manage the long-term effects and risks associated with Hodgkin lymphoma and subsequent treatment. The sensible recommendations in this care plan are designed to help you have productive conversations with your physicians and other healthcare professionals. Because Hodgkin lymphoma is relatively rare, many clinicians may not be aware of the relevant health concerns. Our goal is to update this care plan as research is completed and evidence is available.

We suggest that you print out the document, add your details and share the plan with your physicians. It is intended to:

  • Inform both you and them of the best information we have about the care you should receive over the course of your life;
  • Encourage productive conversations; and
  • Enlighten clinical staff who may not be informed as to why you need to have this plan in place.

This care plan is intended to improve your quality of life by providing the groundwork for you to receive long-term follow-up care and relief of late effects.
